This is the form that your chat support tech asked me to send so he could ask your programmers to create one with the checkboxes already present so the seller can complete it electronically. I've created the boxes twice and my seller checks them electronically, but when I open the envelope, 99% of the boxes are NOT there and are NOT checked.

To confirm, this form is titled "Residential Property and Owners' Association Disclosure Statement 140 - NCR" within the North Carolina Realtors - NCR library?
Can you please send over a screenshot or the question number where the checkbox is located?

In the meantime:

Download the incomplete packet without the seller signatures, but with the other parties.
Use the new copy of the Seller Disclosure from Forms
Upload the incomplete forms that have some parties signatures, excluding the old seller form
Manually place the remaining seller signatures
Send for signature
