I have been having some issues with the forms check boxes. When I assign them to a client to check, they don't see them and the form always comes back with no selections. I decided to try it on myself sending a form with check boxes added for me to check as a signer. When I opened the form to sign, there were no visible boxes to know to check. Also there was no prompt telling me "next" on what to even look for.
